Long Island Family Medical Group

What happened to Long Island Family Medical Group ?  Office is closed with a note on the door.   Phone disconnected. Note says to write to Sayville for your medical records.  My records are locked up in there and no doctor for rewrites of prescriptions.  This is wrong.
