New Kindergarten Registration Process at Miller Place School District

Process for kindergarten registration has changed this year in Miller Place.

The kindergarten registration process in the Miller Place School District has changed this year, and Andrew Muller Primary School is holding registration for one week, March 11 through March 15 from 9:45 a.m. to 3 p.m and it is by appointment only.

In order to register your child, call the school’s main office at 631-474-2715 to schedule a date and time. Registration forms and information are available on the District website. Print and complete these forms and bring them with you along with the required documentation.

Children who are five years old on or before Dec. 1, 2013 and are residents of the Miller Place School District are eligible to enter kindergarten for the 2013-2014 school year.

There are several state mandated immunizations necessary for school entry: three Polio (IPV), three DPT, three Hepatitis, two MMR and one Varivax. You are asked to bring your current immunization record to registration.

New York State law requires that all incoming students have undergone a physical examination on or after Sept. 5, 2012. The completed examination form must be submitted to the Health Office by Aug. 31, 2013.

For more information contact the Andrew Muller Primary School at 631-474-2715 or visit the Miller Place School District website.
